
ONA Transfer Form – RN – FT/PT/JBS – Surgery - General (199092-67)
PLEASE NOTE: ONA Transfer Forms are valid between January 1st and Dec 31st each year, therefore you must complete one each year to be included in this process.
Qualifications:
- Current Certificate of Registration with the College of Nurses of Ontario as a Registered Practical Nurse.
- Current BCLS certification (Level C).
- Must have started Coronary Care 1 within 1 year of start date.
- Must have current or recent clinical experience within the last 2 years in surgical nursing in an acute care setting.
- Demonstrated patient/family teaching skills.
- Demonstrated effective communication skills.
- Demonstrated ability to function effectively in an interdisciplinary team.
- Demonstrated reliable attendance, punctuality and availability.
- Demonstrates a commitment to quality and patient safety, as well as the safety of self and co-workers.
- Demonstrated knowledge of community resources required for effective patient care and discharge planning.
- Evidence of ongoing professional development.
- Demonstrated ability to effectively address / resolve issues including conflict in a respectful and productive manner.

Demonstrated satisfactory performance of the following nursing skills:
- Complex wound care/ Suture & staple Removal; experience with VAC dressings is an asset.
- Vascular Access skills
- Stoma Management
- Blood and blood product administration
- Acute Pain Management strategies
- Enteral and parenteral nutrition
- Nasogastric tube and drain care knowledge
