
ONA Transfer Form - RN - RFT & TFT - Outpatient/Community Mental Health (199181-67)
PLEASE NOTE: ONA Transfer Forms are valid between January 1st and Dec 31st each year, therefore you must complete one each year to be included in this process.
As Case Manager, you will:

Provide clinical assessment (including mental status examination), treatment, skill development,consultation, crisis support, advocacy, and development/coordination of recovery plans specific to client's individual needs and recovery goals

Monitor pharmacological interventions for both efficacy and adverse reactions
-
For assigned clients, monitor the delivery of care through assessment, planning, implementation of treatment and evaluation, and ensure appropriate documentation is completed in accordance with the College of Nurses of Ontario standards and the standards of program

Participate in Clinical Rounds and client conferences to review treatment and recovery plans for individual clients

Complete workload and other statistical information in accordance with Mental Health Ambulatory Services' requirements

Provide liaison and education to community agencies/services, other relevant facilities, and educational institutions including supervision of students

Participate, as appropriate, in research or other evaluation projects
QUALIFICATIONS:
-
Current Certificate of Competence in the Province of Ontario; familiarity with the PACT (ACTT) Model

Minimum three (3) years' experience in a mental health setting providing assessment, treatment, consultation, and education to a similar adult target population; experience in community-based/community outreach service delivery preferred

Demonstrated strong clinical assessment and treatment skills- Demonstrated knowledge of psychiatric medications

Demonstrated knowledge of the Mental Health Act and emerging clinical trends and evidence-based best practice

Demonstrated ability to work in a self-directed manner with minimal supervision

Demonstrated ability to work effectively within a multi-disciplinary team

Excellent interpersonal, organizational and oral/written communication skills
-
Demonstrated willingness to act as an educational and clinical resource to team members, students, clients and community agencies
-
Valid driver's license, reliable vehicle and a willingness to travel is required.

Willingness and ability to work weekends and evenings as required

For the RN position, preference will be given to candidates with University Degree preparation

Demonstrated reliable attendance, punctuality, availability and performance required
