POSITION SUMMARY: 
Joseph Brant Hospital is currently seeking a dynamic Clinical Manager for our Palliative/Transitional/Complex Care Inpatient Unit.  This position will also have leadership responsibilities for the discharge planners, and will lead the Seniors Care portfolio.    Reporting to the Director Medicine, Post Acute and Access & Flow, the successful individual will be accountable for the delivery of a positive patient experience, people management, risk management and safety, and the provision of high quality care. This role will have be oversight of the new Corporate Seniors Care portfolio.  Within this role, the manager will work with various stakeholders throughout JBH to enhance Seniors Care.  This role is ideally suited to an individual with strong clinical knowledge and leadership skills who will promote the delivery of safe, quality patient care, facilitate staff development, and maximize the efficiency and effectiveness of the teams and units.  The Manager is accountable for planning, implementing and integrating plans that are in alignment with the Post Acute Services goals and objectives that align with Joseph Brant Hospital’s vision and strategic directions.  

RESPONSIBILITIES:
The ideal candidate is a strong, innovative leader who excels in leading and working collaboratively in a team environment.   In your role, you will be responsible for the following:

  • The management, operation and organizational effectiveness of the 5 North inpatient units, including managing all aspects of human and financial resources
  • Acting as a coach, mentor and role model to support an environment that promotes cohesiveness, motivation and commitment among your staff through engagement, empowerment, and transformational leadership
  • Supporting staff in problem solving and decision making by ensuring administrative policies and procedures are followed.
  • Working collaboratively with the Program Director and other key interdisciplinary members to address operational issues that impact the delivery of service and the efficient use of resources.
  • Ensuring excellence in patient care through continuous quality improvement processes that include planning, quality monitoring and risk avoidance.
  • Facilitating the use of feedback strategies to maintain and improve performance of team members.
  • Providing leadership in the development of new initiatives by creating an environment that encourages creativity, innovation, adherence to best practice and critical thinking
  • Fostering effective employee and labour relations through knowledgeable interpretation and implementation of hospital policies and collective agreements.

In particular, we are seeking a dynamic leader who has demonstrated the ability to think critically and strategically, build cross departmental relationships, engage stakeholders in design and implementation of change ideas, improve processes, building capacity in others, and sustaining change. Working in collaboration with a motivated team, they will continue to build on the strong processes in place to support hospital discharge planning, ALC avoidance, Home First Processes and Seniors Care. 

QUALIFICATIONS:

  • Registration in good standing with the College of Nurses of Ontario or other regulated Health Professional College
  • Completed Master’s degree or current registration in a Graduate program required (e.g. Health Administration, Leadership)
  • A minimum of three (3) year’s experience in a leadership and/or management position required within a hospital setting
  • Minimum of three (3) years’ experience working in specialized geriatrics across the care continuum
  • Experience in ALC Avoidance Management and Discharge Planning required.
  • Evidence of strong clinical knowledge and decision making
  • Evidence of knowledge of, and experience with, Quality, the use and interpretation of metrics, and change management methodology required
  • Demonstrates excellence in interpersonal communication, coaching, mentoring and team building skills
  • Possess highly developed analytical, problem solving and critical thinking skills
  • Ability to provide support and leadership to advance evidence based inter-professional patient centered care  
  • Excellent interpersonal, communication skills, organizational and time management skills
  • Demonstrated tenacity and strength of character to ensure the efficient and effective completion of all mandates.
  • Demonstrated ability to work as an effective member of a multidisciplinary team.
  • Demonstrated experience in monitoring and managing a clinical budget, financial reporting and analysis.
  • Skilled in staffing, scheduling and optimization.
