Responsibilities:

  • Develop an operational plan for DUE program at JBH in collaboration with the Manager of Pharmacy Services
  • Work collaboratively with prescribers and other health care professionals to design effective medication-use processes.
  • Managing and re-evaluating DUE programs and processes to support ongoing quality improvement
  • Collecting, analyzing, and evaluating patient-specific data to identify, resolve, and prevent medication use related problems.
  • Work with the pharmacy team and the prescribers to add or delete medications from the formulary.
  • Provide information and education based on DUE findings. Identifying areas in which further information and education for health care professionals may be  needed.
  • Review medication utilization on a unit specific level and provide feedback for the unit leadership to reduce wastage and expiries
  • Enhance opportunities, through standardization, to assess the value of innovative medication-use practices from both patient-outcome and resource-utilization perspectives.
  • Minimize prescribing variations that contribute to suboptimal outcomes of medication use by reviewing individual medication orders against Evidence Based Medicine and Best Practice Guidelines and consulting with prescribers and others as needed.
  • Oversee Medication backorders and recall management in collaboration with the Pharmacy Inventory team
  • Provide medication management expertise for the Ontario College of Pharmacists’ yearly accreditation and Accreditation Canada
  • Other duties as assigned by the Pharmacy Manager

The DUE Pharmacist will serve as:

  • Secretary for the Pharmacy and Therapeutic Committee
  • Pharmacist representative on the Order Set approval Committee
  • Pharmacist representative on the Medication Safety Committee

Qualifications:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited university (required)
  • Registration with the Ontario College of Pharmacists, Part A, in good standing (required)
  • 3 years of previous hospital experience is required
  • Experience with Medication Management and Formulary reviews is required
  • Hospital pharmacy residency preferred
  • Meditech computer experience an asset
  • Previous hospital experience an asset
  • Patient centred with attention to details
  • Excellent problem solving skills
  • Proficient in the use of Microsoft Office programs
  • Ability to multitask and work well under pressure  
  • Committed to service excellence and patient safety
  • Excellent verbal and written communication skills
