Position Summary:

Joseph Brant Hospital is seeking a temporary part-time (0.5 FTE) experienced Mental Health and Addictions (MHA) Registered Nurse. This position is part of an exciting quality improvement collaborative to implement Seamless Care Optimizing the Patient experience (SCOPE) in Burlington. The SCOPE RN Navigator, will work within recovery and guided self-help models of care. Supporting community based Primary Care Providers (PCP) through a single point of access, the SCOPE RN Navigator facilitates timely connections to MHA care and navigates to services available in the community.

Reporting to the Manager MHA Services, the SCOPE RN Navigator, specializes in MHA systems navigation. The SCOPE RN Navigator will work collaboratively with primary care providers to assess, triage, support and facilitate timely navigation to appropriate out-patient hospital and community based mental health & addictions services. This role is ideally suited for an experienced Mental Health & Addictions Registered Nurse with diverse and expert skills in mental health & addictions assessment of needs, ability to triage for acuity, diagnosis and treatment recommendations, system navigation (walking with), and facilitate therapeutic interventions as a bridge to more appropriate treatment.

Responsibilities:

In this role you will work closely and collaboratively with Burlington Primary Care Providers, the SCOPE RN Navigator will be responsible for:

  • Assessing, triaging and navigating patients (non-urgent) to the most appropriate MHA service (in-person and virtually)
  • Formulating recommendations or treatment plans;
  • Implementing an intervention plan with patients, family members, significant others, and the interprofessional team for the purpose of facilitating changes in behavior, attitudes, feelings and/or the environment;
  • Facilitating family/team meetings; reassessing and modifying the intervention plan as required
  • Liaising with community family physicians and appropriate hospital and community agencies (in person and virtually), as clinically required to facilitate warm transitions in care.
  • As a member of the SCOPE Program, the individual will contribute to the development and review of this integrated primary care model, including appropriate data collection, reporting, analysis and model development

Qualifications:

  • BScN required
  • Master’s (Leadership, Nursing) required
  • Current registration in good standing with the College of Nurses of Ontario
  • Five (5) or more years experience working within an inter-professional team in a MHA acute care setting and/or primary care setting required.
  • Experience working with equity-deserving populations with complex health service needs required.
  • Strong knowledge of hospital and community mental health system services and resources.
  • Ability to work independently and autonomously required, with a track record of excellent attendance, punctuality, and work performance.
  • Excellent verbal and written communications skills.
  • Excellent interpersonal, time management and organizational skills.
  • Excellent problem solving and decision making skills.
  • Knowledge of policies, legislation, programs and issues related to social welfare institutions and Services preferred.
  • Must be flexible and able to adapt to changing environments and travel outside of hospital and within community.
  • Ability to adapt to a changing environment and assume responsibility for own learning.
  • Understanding of psycho-social functioning.
  • Ability to diffuse conflict and foster collaborative working relationships.
  • Self-directed, organized, and diplomatic.
  • Experience with quality improvement an asset.
  • Valid driver’s license and reliable mode of transportation.
